The Children’s Community Nursing Team (CCNT) provides home nursing services for children and young people (CYP) in Trafford who have mild, short term,  acute illness or chronic/longterm, complex and additional needs.

 

Our service aims to:

• Deliver nursing care closer to home for children and young people who have mild acute illness, long term chronic health needs, or complex/palliative care requirements
• Reduce avoidable hospital admissions for  children and young people
• Facilitate early discharge from hospital after elective and non-elective admissions

 

Referral Methods

• Self referral
• GP
• Health professional
• Social care professional
• Referrals are accepted from any agency working with the child and family provided permission is sought from the parent/carer and the child has a diagnosed condition
